She asserts that gender identity confusion among youth is largely a social phenomenon influenced by family trauma, mental health challenges, and cultural factors rather than a biologically determined condition. Dr. Cretella, a board-certified pediatrician, traces this trend to increased exposure to gender-nonconforming media and social pressures, amplified by social media, which she argues has caused a sharp rise in cases, especially among adolescent girls. She connects this trend with similar patterns observed in other body-image-related disorders, such as anorexia.

Dr. Cretella critiques the affirmative care model used in pediatric treatment, which advocates for social and medical affirmation of a child’s identified gender. She claims that encouraging early social affirmation, puberty blockers, and cross-sex hormone treatments creates a “self-fulfilling prophecy,” leading more children to pursue medical transitions. Dr. Cretella contends that puberty is a natural period for many children to resolve gender dysphoria, but by inhibiting this process, physicians could be locking children into irreversible medical paths.
